Case Management Specialist
Job Description:
- Ensure that the patient receives the best possible care by coordinating with doctors, nurses, hospital administration, and external service providers.
- Link between different professionals to identify gaps in care plans using communication and available resources to promote patient safety, quality of care and cost – effective outcome.
- Review the examinations, procedures and medical consultations required for patients, and make the necessary communications with the relevant employees to improve the quality and speed of the medical service provided.
- Review the patient's medical record to ensure that his or her care plan is documented and followed accurately.
- Create a daily list of “high priority patients” to be closely monitored by the case management team.
REQUIREMENTS:
Bachelor degree in pharmacy or dentistry.
0-2 years experience in healthcare field. preferably hospital patient experience or insurance
Excellent commend in using Microsoft Office
Excellent commend in English Language
Strong organizational and communication skills
Ability to work collaboratively in a team environment
