Job DescriptionReview patients' medical histories, laboratory results, and other relevant information to identify drug-related problems, contraindications, and potential interactions.
Monitor patients' response to medications, assess therapeutic outcomes, and make adjustments as necessary to achieve optimal therapeutic outcomes.
Provide drug information and clinical guidance to healthcare professionals to promote safe medication practices and prevent medication errors.
participates in writing or evaluating Medication therapy protocols, policies and procedures.
Provide guidance on lifestyle modifications, drug interactions, and potential drug-food interactions to optimize medication outcomes.
Developing a pharmaceutical care plan.
Job Requirements
Bachelor's degree in clinical pharmacy.
(PharmD) degree from an accredited pharmacy program.
2 - 4 years as clinical pharmacist .
Strong clinical knowledge and understanding of pharmacotherapy principles, drug interactions, and medication safety practices.
Attention to detail and accuracy in medication order review, dosage calculations, and documentation.
